At least 40% of women experience urinary incontinence. Often the incontinence gets worse over time. Many women suffer quietly with this problem not knowing that there are treatments available. There are several different types of urinary incontinence. We offer in office evaluation for incontinence. Evaluation begins with a series of questions about the specific symptoms, a pelvic exam to evaluate pelvic floor support and muscle strength as well as a urine analysis and culture. If symptoms are moderate to severe additional evaluation with a bladder diary and urodynamic testing is done. Urodynamic testing helps us identify the specific type of incontinence (stress, urgency or mixed) and the location of the anatomic problem.

Urodynamic testing is done by placing a very small catheter/transducer inside the bladder and another transducer inside the vagina. The transducers measure pressure electronically as the bladder is slowly filled with fluid. The pressure at which the bladder leaks and how it leaks is determined.

Treatment for urinary incontinence varies with its type and severity. Urge incontinence (or unstable bladder) can be treated with biofeedback, topical estrogen and medication such as Detrol. Stress incontinence can be treated with daily Kegel exercises, physical therapy, biofeedback, continence pessaries and surgical repair. Surgical repair may include a bladder suspension, a urethral sling, and pelvic floor reconstruction for prolapse. The type of treatment depends on the type of incontinence, the anatomic findings, the severity of symptoms and patient choice.

It is important to remember that urinary incontinence is a common problem that can be treated. Incontinence is not a natural part of aging.
