We require payment at the time of service for all office visits and minor procedures. We provide you with an itemized bill for you to submit to your insurance company for reimbursement. We will directly bill your insurance company for covered office visits and procedures if your physician is a participating provider. We also bill directly for all patients' covered hospital surgeries; for those fees not covered, a reasonable payment plan will be arranged.
Patients who subscribe to health plans for which our physicians are not participating providers are welcome to be seen at Avenues For Health. Those patients may need to exercise their out-of-network option, if available to them, for their visits to Avenues For Health doctors, in order to be covered by their health plan. This choice may increase the patient's personal financial responsibility for care provided. We will gladly help you investigate your insurance situation in advance of any major procedure.
The physicians at Avenues For Health belong individually to a variety of health plans. We change these memberships periodically in response to changing participation of community businesses in these plans, with the goal of providing continuity of medical care for our patients.
